IOS incorrect password

I have EAP 115 and 225 installed at few sites, currently I facing some problem on certain IOS devices, those IOS devices been connected to the SSID before, but some of the time, it will showing the connection to the SSID was incorrect password, have to forget the SSID and reconnect again only can connected.
It only happen in IOS devices and I cannot found any log on EAP controller.
