May I know the controller you are using now? Are you using the software controller? 

For the fail to send command error, generally it indicates that the communication between the controller and EAP is blocked. You can try to test whether the EAP can work properly in standalone mode first. And please try to ping the EAP IP address from the controller.

Besides, please also try to disable the antivirus/firewall on the computer or use another computer and try again.
