Disabled Mesh but EAP's are Cascading
I've disabled Auto-Failover and then Mesh but I'm still getting cascading EAPs? Tried rebooting all EAPs and Force Provisioning. Other ideas or is this a problem with the latest firmware of the controller?

Probably not the most elegant solution but turning OFF EAP LLDP does in fact show the map correctly.

Additional information:
In the Details Tab of the Device/Downlink it shows an EAP downstream. However, in the Mesh Tab (had to temporarily turn on Mesh to see info) of the Device it says "No Entry in Table". Seems to be a bug...?
Also, all EAPs say Uplink (wired)...

How are the EAP units connected with each other? via a switch or somehow? This may help us understand the situation better.

@Vincent-TP They are all connected to a TL-SG1005P (to get PoE) behind the main switch that is a TL-SG2218. Thanks!!

Did you try to reboot the switch SG2218.
Please reboot the switches in this order:
Controller >> Router >> SG2218 >> SG1005p >> EAPs

@Vincent-TP Seemed to work for about a minute after rebooting (all EAP's were shown at the root level). However after this they whent back to the cascade format... Below is after rebooting and waiting for a few minutes.

If you have a non-Omda switch in your network, something like this will happen. You won't get a proper map, sometimes the network will split up completely, so I recommend you replace the TL-SG1005P with a SG2008P if you are concerned about the map being correct.
Here is an example of what can happen when there is a non-Omada switch between the router and access point.
