I found something odd on my system. The adapter can only be attached into PCIe 8x Wired (run as 4x). It's solved!!

I have manually installed the latest driver downloaded from here (I uninstalled the previous driver first). Then it produces BlueScreen IRQL_NOT_LESS_OR_EQUAL. Another indication of driver problem.
