After upgrading my OC200 v1.0 hardware controller to firmware 5.4.7 my T1600G-28PS v3.0 switch shows disconnected. Switch is not accessible, doesn't respond to ICMP, and is not servicing any connections outside the management VLAN. Force provision does not readopt the device and a power cycle doesn't fix the issue. Router, Controller, Switch, and WAP are all on the same management VLAN. I don't recall seeing any compatibility issues in the release notes.

 

What should I do? Do I need to force a factory reset?

 

Edit: From the controller I forgot the switch device. Forced a factory reset which allowed me to readopt successfully. FWIW, you will loose your port naming, port profiles, LAGs, and static IP address information.
