After I plug it in, all four lights flash and it stays that way.

The extender seems to have a defect on the hardware, you could try a different socket and a factory reset, if still the same case, it is recommended to check warranty with local support or retailers.
RESET: With the extender powered on, use a pin to press the RESET button until most Leds flash once.
