Is there a way to have a separate SSID for each of the two WiFi bands? So far in the app I can only find a way to change the SSID, which is used by both bands

You have SMART CONNECT enabled... it is a Wi-Fi setting. Both 2.4Ghz and 5Ghz radio's will use the SSID and P/W set on the 2.4Ghz band.
Turn it off and you can set each one individually.
