Hi, when it is offline, have you checked whether the adapter is still properly detected and ready to use?

If possible, please also change another PCI-E slot on Windows 11 to see whether it helped.
It is also suggested to install the driver here to see whether it helped or not:

@David-TP I hit the same issue but resetting it now in windows safe mode not working anymore. I use the same driver you linked and it doesn't work.
